of Patient Privacy Rights
As a
home health patient, you have the privacy rights
listed below.
- You have the right to know why we need
to ask you questions
- You have the right to have your personal
health care information kept confidential
- You have the right to refuse to answer
questions
- You have the right to look at your
personal health information
- You can ask the Center for Medicare
and Medicaid (CMS) to see, review, copy, or correct
your personal health information, which that Federal
agency maintains in the HHA Oasis System of Records.
You can contact the CMS by calling toll free, 1-800-MEDICARE.
In addition,
you have a "Patient's Right to Choose" the home
health organization of your choice. The Balanced
Budget Act of 1997 guarantees that patients have
the right to choose their home care provider when
they are discharged from a hospital at which time
they should be given a list of Medicare-certified
home care agencies to choose from. VNA-TIP
HomeCare has requested
to be included on the list of home care agencies
given to patients at the following hospitals:
